Lost or Damaged Passports

An Indian national resident in Zambia who has lost or has a damaged passport should apply for a new passport.

Documents required: 

  1. Photo specification for passport application (Click here)
  2. Applicant may apply at: https://embassy.passportindia.gov.in/ and complete the application online after carefully going through the instructions listed on the web page.

  3. Copy of the duly filled online form and submit a signed copy.

  4. Four recent Passport size colour photographs (see ‘Photograph Guidelines’)

  5. Passport with photocopy of pages 1,2 and last page of the passport including copy of endorsement if any on the passport (original and photocopy).

  6. Residence permit (original and photocopy)

or

Work permit holders: Letter from company (original and photocopy)

  1. One Personal Particular Form (PP Form)duly filled in and signed with photos pasted in the top right-hand corner. Please note that for question 8(a), 10(1) and 10(2), you must provide an address in India. It is important to mention the police station and pin code too.

  2. Police report intimating loss of passport (original and photocopy).

  3. Newspaper clipping intimating loss of passport (original and photocopy).

  4. Declaration (Annexure-F)

  5. A signed statement explaining the circumstances under which the passport got lost/damaged

Minor applying for passport in lieu of lost/damaged passport

  1. All above documents 1 to 8; and

  2. Declaration signed by parent(s) (Annexure-F)

  3. A signed statement by parent(s) explaining the circumstances under which the passport got lost/damaged

  4. Passport of parent(s) and residency/work permit (original and photocopy of pages 1,2 and last page including any observation).

  5. Declaration by both the parents (Annexure-D)

or

Declaration where one parent has not given/is not able to give consent (Annexure-C)

or

Declaration by guardian of a of minor applicant (Annexure-D1)

Time frame: 10 - 15 days (if passport is issued from Lusaka or passport particulars are available online)

(to be paid in cash)

ZMK 3350/- (Ordinary)
ZMK 3900/- (Jumbo)

Where to apply:  Directly at the High Commission of India in person

A)  An adult applicant should submit the application in person

B)  In case of a minor applicant

i) When both parents accompanying the minor (Annexure-D)

or

ii) When either of parents accompanying the minor (Annexure-C)
